Care Coordinator

Job Description

We are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in helping individuals with intellectual and developmental disabilities achieve their long-term and short-term goals by developing, implementing, and monitoring personalized Life Plans.

You will work closely with members, interdisciplinary teams, and other critical people in their lives to conduct meetings and assessments, ensuring comprehensive person-centered Life Plans. You will also ensure integration of all needed and preferred supports and services, and maintain ongoing contact with members.

Your exceptional organizational skills, strong communication abilities, and passion for delivering high-quality service make you an ideal candidate for this dynamic position.

Required Skills and Qualifications
  • Bachelor's degree with 2+ years of relevant experience or equivalent qualification.
  • Strong communication skills, including verbal and written communication, interpersonal, and organizational skills.
