The Opportunity:
We are seeking a skilled and experienced Clinical Care Coordinator to join our team in a pivotal leadership role at a Residential Aged Care Home.
This is an ideal opportunity for someone with strong change management skills and a genuine passion for uplifting people, culture, and care outcomes.
- Organising and facilitating case conferencing to partner with consumers and their representatives in the development of care provisions and plans;
- Provide expert case management alongside the Residential Care Manager
- Monitor and improve clinical outcomes through audits and quality initiatives
As a Clinical Care Coordinator, your key responsibilities will include:
- Care Planning and Coordination: Organising and facilitating case conferencing to ensure that care provisions and plans meet the needs of our residents.
- Case Management: Providing expert case management alongside the Residential Care Manager to ensure seamless delivery of care services.
- Clinical Outcomes: Monitoring and improving clinical outcomes through regular audits and quality initiatives.
To be successful in this role, you will need:
- Strong Leadership Skills: Proven ability to lead and manage teams effectively.
- Change Management: Experience in implementing and managing change within complex organisations.
- Collaboration: Ability to work collaboratively with consumers, families and healthcare professionals to achieve positive outcomes.
