We are seeking a Registered Nurse to join our Chronic Care Disease Management team. Your role will involve providing care coordination, case management and support to individuals with chronic disease, their carers, General Practitioners, practice staff and staff involved in the provision of care.
You will work closely with relevant stakeholders to ensure coordination of services, utilizing shared care plans and other team care approaches to optimally support individuals with chronic disease within the framework of the Chronic Disease Management service.
About The Role- Care Coordination: Work with individuals with chronic disease and their families to develop personalized care plans, ensuring seamless transitions between healthcare providers.
- Case Management: Provide comprehensive case management services, including assessment, planning, implementation, and evaluation of care for individuals with complex needs.
