Holistic Nursing Professional

We are seeking a dedicated Community Care Nurse to deliver high-quality holistic nursing care in the Mandurah, Falcon and Greenfields area.
The ideal candidate will possess excellent clinical skills, strong communication abilities and a passion for community care.
Key Responsibilities:
- Clinical in-home services including care plan assessment, medication management, wound care, chronic disease management and palliative care.
- Supervising non-clinical team members in a community setting.
- Intake processing, clinical care planning and reporting.
- Conducting client care assessments with a focus on Consumer Directed Care.
- Coordinating the provision of quality person-centred care according to client needs and service/care plans.
Applicants must have:
- Current AHPRA registration as an Enrolled Nurse.
- First Aid & CPR certification.
- Australian National Police check.
- Professional Indemnity Insurance.
- Valid Driver's Licence and reliable presentable car.
- A smart mobile phone (iPhone / Android).
While not essential, it would be desirable for applicants to possess:
- A passion for high-quality client-focused care in a community setting.
- Cert IV in Training and Assessment.
- Exceptional communication skills.
- Ability to work autonomously or within a team environment.
- Excellent time management and organisation skills.
