We are seeking a skilled clinician to join our multidisciplinary team, delivering frontline services to individuals facing complex challenges.

The role involves conducting comprehensive assessments and developing care plans tailored to each patient's needs. You will provide assertive follow-up support, including home visits, to promote recovery and long-term wellbeing. Your clinical expertise will contribute to shaping procedures, strengthening referral pathways, and enhancing team operations.

  • You will have tertiary qualifications in Social Work, Occupational Therapy, or Rehabilitation Counselling.
  • Membership with the Australian Association of Social Workers or AHPRA registration as an Occupational Therapist is required.
  • Clinical skills in assessment, planning, and evaluation of psychosocial interventions are essential, along with independent professional judgment on routine matters.
  • You must have experience with advanced reasoning in patient assessments, report writing, and developing/reviewing care plans.
  • A commitment to professional and ethical boundaries is crucial when working with a medically led treatment team.
  • Knowledge of social and emotional needs related to trauma, grief, loss, domestic violence, and child protection is vital.
  • Experience in working with Aboriginal patients and improving health outcomes for disadvantaged groups is highly valued.
