As a Care Manager, you will be responsible for overseeing clinical operations and conducting in-home assessments to create personalized care plans that meet the needs of our clients.
Key Responsibilities- Ensure quality care delivery by implementing Right at Home policies and adhering to relevant standards.
- Develop and implement effective care plans that address the unique needs of each client.
- Lead and manage staff on clinical competencies, fostering a culture that promotes excellent care delivery.
- Monitor performance, identify areas for improvement, and implement changes to drive continuous quality enhancement.
- Health Science degree (essential); postgraduate or specialist certificates (desirable).
- Demonstrated knowledge of Aged Care Quality Standards and continuous quality improvement principles.
- Current registration with AHPRA as an RN, Physio, or OT.
- Valid driver's license, comprehensively insured vehicle, and police clearance.
- Minimum 3 years' experience in aged or community care.
- Proven ability in assessment, care planning, and team leadership.
- Excellent interpersonal, communication, and problem-solving skills.
