We are seeking a highly motivated and compassionate individual to join our team as a Local Area Coordinator. As a key member of our community team, you will play a vital role in empowering people with disabilities to live independent and fulfilling lives.
Key Responsibilities- Support individuals with disabilities to develop personalized plans that align with their strengths, skills, and interests.
- Guide and assist individuals with disabilities to build capacity and explore options for self-direction, self-management, and self-advocacy.
- Provide individuals with disabilities and their families with information and support to access mainstream services and participate in the community.
- Build and maintain positive partnerships within the community to refer individuals to a diverse range of community and other government services.
- Develop strong community partnerships that support capacity building to make communities more accessible and inclusive.
- A genuine desire to empower people with disabilities and a modern approach to community development.
- Relevant qualifications in disability, allied health, or community services, or lived experience with disability or caring for someone with disability.
- Demonstrated ability to meet and exceed key performance indicators and targets.
- Excellent communication, listening, and relationship-building skills.
- Proven ability to work collaboratively with a wide range of team members, in person and virtually.
- IT savvy, with experience using CRM, Microsoft suite, and other relevant software.
- A person-centered approach to outcomes, confidentiality, and professional integrity.
- Ability to travel extensively, meeting with individuals in their homes or community settings.
