We are seeking an experienced and motivated Clinical Care Coordinator to provide high-quality care to residents in our residential aged care facility.
The OpportunityThis role involves working with a progressive team to deliver safe and effective clinical care to individuals, including complex wound care, pain management, behaviour management, end-of-life care, and other clinical procedures.
Key Responsibilities Include:- Undertake or supervise clinical care for individual residents as required, including coordination of care plans and liaison with healthcare professionals.
- Liaise with residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive comprehensive care.

- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia administered through the Australian Health Practitioner Regulation Agency.
- Recent experience in caring for high-care and dementia residents.
- Knowledge of wound care, palliative care, behaviour management, clinical assessments, and care plans.
A valid Police Clearance or National Disability Insurance Scheme (NDIS) Worker Screening Clearance is mandatory when working in the Aged Care sector.
It is also recommended that all employees be vaccinated against Covid-19 (3 vaccines) and receive the Flu-Vaccine.
