Care Coordinator

As a Support Coordinator, you will work closely with recipients to navigate their individual NDIS plans. The aim is to ensure they get the most out of their plans.
">Key Responsibilities Include:- Empowering recipients to make choices about how they want to live their lives.
- Coordinating services for recipients to support them in living fulfilling and meaningful lives within the community.
- Engaging with stakeholders, such as government agencies, service providers, and families, to leverage supports and services effectively.
- Helping recipients strengthen their ability to coordinate their own supports.
- Certificate IV in mental health or minimum 2 years' experience in the sector.
- Current nationally recognized First Aid and CPR certificates.
- A valid Queensland driver's license.
- NDIS Worker Screening Clearance.
- NDIS Worker Orientation Module 'Quality, Safety & You' completion certificate.
- An hourly rate of $43.23+ super.
- Up to $15,900 FBT salary packaging benefits.
