This is an excellent opportunity for a highly skilled and dedicated Enrolled Nurse to join our team as a Community Health Care Professional. As a key member of our community health care team, you will be responsible for delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Visit multiple clients in the community each shift (typical caseload: 8-10 patients) to provide personalized nursing services.
- Perform wound care, wound assessments, and regime changes to promote optimal patient outcomes.
- Administer and manage medications safely and effectively.
- Provide continence and stomal care; conduct aged care nursing assessments to identify areas for improvement.
- A genuine passion for nursing and providing high-quality patient care in the community.
- An Enrolled Nurse with unrestricted AHPRA registration and at least 2 years of experience delivering community health care services.
- The ability to work independently and as part of a team to achieve outstanding patient outcomes.
- Excellent communication and interpersonal skills to build strong relationships with clients and colleagues.
Nurses work on a rotating 7-day roster between 7am and 9pm, including morning (AM) and afternoon (PM) shifts with no night shifts. Management supports scheduling preferences where possible, with potential for alternative times if needed.
