Job Title: Clinical Pharmacist

We are seeking a highly motivated and dedicated Clinical Pharmacist to join our team at Icon Group. As a Clinical Pharmacist, you will play a crucial role in providing high-quality pharmacy services to our patients.

About the Role:

The successful candidate will be responsible for coordinating and organizing the supply of chemotherapy for oncology inpatients and day oncology unit patients within our clinics. You will also develop and utilize your clinical skills to improve the outcome for cancer patients, perform medication chart reviews and medication reconciliation, record clinical interventions, build relationships across multidisciplinary teams, educate staff of Slade Pharmacy, and dispense and supply medication and pharmacy products.

Key Responsibilities:
  • Coordinate and organize the supply of chemotherapy for oncology inpatients and day oncology unit patients within our clinics.
  • Develop and utilize your clinical skills to improve the outcome for cancer patients.
  • Perform medication chart reviews and medication reconciliation.
  • Record clinical interventions.
  • Build relationships across multidisciplinary teams.
  • Educate staff of Slade Pharmacy.
  • Dispense and supply medication and pharmacy products.

Requirements:
  • AHPRA registered pharmacist with a minimum of 2 years of Australian based pharmacy experience.
  • Team orientated attitude, a focus on delivering exceptional service and care to patients, and the ability to manage your own workload with good communication skills to effectively liaise with a wide range of health professionals.
  • Highly driven and have high attention to detail and a passion for building on your clinical skills and knowledge.
