Job Title: Family Support Specialist
About the Role:The role of a Family Support Specialist involves providing case management support and services to vulnerable children and young people from pre-birth up to and including 17 years of age, and their families. The primary objective is to address the developmental needs of these children and young people who are at risk due to cumulative harm and/or experiencing difficulties.
Key Responsibilities:- To undertake assessments of the needs of children, young people, and their families.
- To develop Child and Family Action plans by partnering with the family and other services to provide an integrated service within the Best Interests Case Practice Framework.
- To deliver evidence-informed best practice programs and services, as directed by the Leadership Team, in line with strategic objectives for the Family and Children's Services teams.
Requirements:
Deliver services in an accountable, professional manner and with superior customer service, in accordance with legislation, guidelines, and duty of care requirements.
Provide parenting support, skill development, referrals, and community supports and services in line with the Best Interest Interests Framework.
Plan, develop, and monitor the implementation of evidence-informed interventions.
Maintain, review, and report on case load and service delivery addressing service improvements with support and guidance to ensure full compliance, safety guidelines, targets, and high standards of record keeping.
Engage with the day-to-day operations of the team including caseload management, performance management, professional development with guidance and support as required.
Support and maintain effective positive relationships within the organization and externally.
Demonstrate a commitment to ensuring risk management and monitoring obligations are fully met.
