Responsibilities:
  • Conduct comprehensive assessments of patients' physical and functional abilities to understand their needs.
  • Develop and deliver individualised or group treatment plans based on patients' needs, goals, and preferences.
  • Implement evidence-based physiotherapy interventions to address mobility issues, pain management, and other age-related conditions.
  • Regularly assess and report on patients' physical and psychosocial capabilities to care management teams.
  • Maintain, prepare, and support precise records, exercise sheets, assessment reports, and progress notes that accurately reflect patients' physical function.
  • Make recommendations regarding exercise programs, mobility equipment, safety at home, and refer to other disciplines as required.
  • Uphold ethical and professional standards in accordance with regulatory guidelines.
  • Collaborate with interdisciplinary teams to provide holistic care.
  • Educate patients and families on preventive measures and exercises to improve and maintain mobility.
  • Participate in team meetings, case conferences, and training sessions to contribute to continuous improvement of care delivery.
  • Engage with team members, patients, and families in a positive and respectful manner, welcoming a culturally diverse team.
To be successful in this role you'll need
  • Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to manage all aspects of compliance.
  • Demonstrated experience with multidisciplinary teamwork.
  • Effective time management to ensure patient intervention, reports, and outcomes are achieved in a timely manner.
  • Ability to prioritise and organise own work/caseload.
  • Demonstrated computer literacy.
  • Demonstrated knowledge and experience of health, aged, and/or disability services.
  • Current Driver's licence and registered and insured motor vehicle.
