About the Role

We are seeking a dedicated and compassionate Aboriginal Community Wellbeing Worker to join our team at Central Australian Aboriginal Congress.

The successful candidate will provide a range of services, including outreach to clients, planned transports, service brokerage, cultural input to service delivery, client support for healthcare follow-ups, healthcare advocacy, client follow-ups for missed appointments, and short-term case management.

This role is critical for the promotion of initial and continued access to holistic primary healthcare.

As part of this role, you will be supported to attain qualifications in an area such as alcohol and other drugs, community services, or mental health.

Key Responsibilities
  • Deliver culturally sensitive and holistic primary healthcare services to Aboriginal clients
  • Provide outreach and support services to clients in their homes and communities
  • Work collaboratively with healthcare professionals and other stakeholders to ensure comprehensive care
  • Develop and implement individualized care plans to meet the unique needs of clients
  • Conduct health education and promotion activities to improve health outcomes
