Key Responsibilities
- Conduct thorough psychosocial assessments to identify social determinants of health and develop individualized plans of care
- Collaborate with patients, families, and healthcare teams to facilitate timely and safe discharges
- Utilize advanced conflict resolution skills to resolve delays and obstacles to discharge
- Document all activities and communication in the medical record and hospital-wide information systems
Qualifications
- LCSW licensure in Illinois
- Master's Degree in Social Work from a CSWE-accredited program
- Minimum of 2 years post-graduate experience in hospital Social Work or related settings
- Excellent interpersonal and organizational skills
