Job Description

We are seeking a skilled Coordinator to lead and energize teams in providing exceptional service. This role requires strong client relationship management skills, as well as the ability to develop and manage care teams that foster choice, control and confidence.

The successful candidate will be passionate about delivering client-focused care and working collaboratively with clients and care teams. They will have excellent communication and interpersonal skills, as well as the ability to inspire and support other team members.

About the Role

This is a unique opportunity to work in a dynamic and innovative organization, contributing to the development of our clients' goals and outcomes. The Coordinator will be responsible for leading a team of Assistants, coaching and mentoring them, and developing positive relationships with clients and their families.

The ideal candidate will have a successful track record in disability, health or community services, including experience coordinating a team and managing client records. They will also possess relevant qualifications and/or experience in Community Services/Case Management.

Key Responsibilities
  • Lead a team of Assistants to provide great service

  • Develop and maintain positive relationships with clients and their families
  • Work collaboratively with colleagues to achieve shared goals
  • Monitor and report on client progress and outcomes
