A Competitive Salary PackageWe offer a highly competitive salary package that includes a base salary of AU$90,000 - AU$110,000 per annum, depending on experience. Job DescriptionWe are seeking an experienced Healthcare Navigator to join our Health Independence Program (HIP) team. The successful candidate will be responsible for managing chronic disease cases, coordinating patient care, and providing education and support to patients and their families.
The ideal candidate will have a registered nurse qualification, experience in cardiac or pulmonary nursing, and excellent interpersonal skills. You will work closely with our multidisciplinary team to develop and implement individualized care plans that promote effective management of chronic diseases.
