Job Title: Support Worker

 

We are seeking an enthusiastic and caring individual to join our team as a Disability Support Worker. As a key member of our team, you will provide support and care for clients with complex mental and physical disabilities in their own homes.

 

Key Responsibilities:

• Provide personal care and domestic duties to clients with complex needs

• Assist clients with medication administration

• Develop and implement person-centred care plans

• Maintain accurate records and reports

• Work collaboratively with other healthcare professionals to ensure seamless care

 

Requirements:

• Certificate III in Individual Support (Disability) or higher

• Experience working with people with disability who have support needs, particularly those with mental health issues

• Previous skills and qualifications in medication assistance and/or administration

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Availability to work flexible hours, including evenings and weekends
