Social Worker HP3/HP4 Job Description

We are seeking an experienced Social Worker to join our team at Sunnybank Private Hospital, a 122-bed facility located on the Southside of Brisbane. As a Social Worker, you will play a vital role in providing supportive and coordinated guidance to patients across Inpatients and Outpatient Day Therapy Programs.

Key Responsibilities:
  • Work independently with patients and as a multidisciplinary team member to provide Social Worker support.
  • Provide guidance and support to patients across the Inpatient Medical, Surgical, Rehabilitation, and CCU wards.
  • Liaise with local support providers to meet the needs of patients discharging from hospital.
  • Assist team members with complex discharge planning needs.

Selection Criteria:
  • Appropriate tertiary qualifications in Social Work.
  • Eligibility for membership of the Australian Association of Social Workers.
  • Ability to work autonomously (sole therapist).
  • Excellent organisation, time management, and self-management skills.
  • Highly developed communication skills and interpersonal skills.
  • Ability to work within and provide leadership within a multidisciplinary team.
  • Commitment to ongoing education and the provision of evidence-based practice.
  • Sound knowledge of local service provision, including those through My Aged Care and NDIS.
  • Knowledge of hospital operations and processes, including discharge planning.
  • Knowledge of EPOA, QCAT, adult guardian requirements, and responsibilities.
