Job Title: Community Support Specialist

Deliver high-quality, person-centred support under the Commonwealth Home Support Program (CHSP) and the Home and Community Care Program for Younger People (HACC-PYP).

Our ideal candidate will be passionate about delivering care and supporting older people and younger people with disabilities.

Key Responsibilities:
  • Domestic Assistance: Assist clients with household tasks such as cleaning, laundry, mopping, vacuuming, shopping, and light meal preparation to ensure a safe and comfortable living environment.
  • Personal Care: Provide assistance with showering, bathing, dressing, grooming, medication prompting, and general hygiene needs delivered with dignity and respect.
  • Individual Social Support: Offer companionship, assist with shopping, transport to essential appointments, and help clients remain connected to their community.
  • Respite Care: Provide in-home or community-based respite for carers while ensuring clients remain safe and engaged.
  • Wellbeing Monitoring: Observe and report changes in clients' health and wellbeing to support independence and safety.
  • Team Contribution: Collaborate with colleagues, attend staff meetings, and participate in training and development.
  • Administration: Maintain accurate and timely records to ensure quality service delivery and compliance.

The successful candidate will be physically fit, able to perform everyday tasks such as vacuuming, lifting, and practical household support. They will also have knowledge of CHSP and HACC-PYP principles and hold a valid Victorian Driver's licence and access to a safe and appropriately registered vehicle.
