About the Role
  • Our organisation seeks a highly skilled and dedicated professional to provide case management support to individuals who are homeless or at risk of homelessness.

The successful candidate will work closely with participants to develop, action and review case management plans to meet their individual needs. This includes providing practical and personal support, crisis budgeting, information, advice and community referrals.

Key Responsibilities
  1. Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as needed.
  2. Provide respectful, proactive, persistent, reliable, practical and comprehensive support that upholds the dignity and privacy of individuals.
  3. Maximise participants' capacity for independent living.
  4. Advocate and assist participants to access housing and specialist services such as drug or alcohol, mental health, legal, domestic and family violence services.
Required Skills and Qualifications
  • Certificate IV qualifications in Community Services or similar.

The ideal candidate will have a passion for making a difference in the lives of individuals and be able to build and maintain relationships with various stakeholders. They will also have experience working within a high-performance framework with accountability for key performance indicators on program delivery and outcomes, in a fast-paced solutions-focused environment.
