Aged Care Needs Professional

We are seeking a dedicated and compassionate Aged Care Needs Professional to join our team.
About the RoleThe successful candidate will be responsible for conducting home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
Key responsibilities include establishing goals specific to the client, developing appropriate strategies and actions, including generating codes for services for referral to age care and non-aged care services.
The role involves facilitating referrals and developing Support Plans for clients.
Additional duties include completing the IAT and matching and referring clients to appropriate service(s).
Flexibility regarding the location of the role may be required depending on client needs.
Requirements- Well-developed written, verbal and interpersonal communication skills.
- Demonstrated computer skills with capacity to learn and use electronic reporting systems.
- Demonstrated ability to work independently as well as effectively in a collaborative team environment.
- Demonstrated equivalent experience working with older people and/or Certificate III in Home Care or Aged Care or Allied Health Assistance.
