Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our dedicated team at Integratedliving Australia. As a Social Worker, you will play a vital role in promoting the wellbeing of our aged and vulnerable clients, enabling them to live independent lives in their own community.
Key Responsibilities- Conduct initial and on-going assessments of client's circumstances, making recommendations to resolve complex and unique situations.
- Manage a portfolio of mixed clients with diverse needs, including dementia, housing, access to services, guardianship, or elder abuse.
- Work collaboratively as part of the multidisciplinary team to provide holistic care to clients and liaise with their carers, case managers, and other health partners.
- Conduct health assessments to evaluate client's needs and create plans, recommendations, and take corrective actions.
- Have the opportunity to contribute to our Wellness for Independence group programs and promote health and wellbeing in the community.
- Bachelor Degree or higher in Social Work and AASW membership.
- Satisfactory Police check.
- Current First Aid and CPR.
- Current driver license and ability to travel for client care.
- Highly effective communication and interpersonal skills.
- Well-developed IT skills and experience with client management software.
- Experience working with a multidisciplinary team and exposure to collaborative practices.
- Sound clinical knowledge and ability to deal with complex issues.
