Transitioning Support Professional

We are seeking an experienced nursing professional to join our team and contribute to the delivery of in-home support services under the Transitional Aged Care Program framework.
Key Responsibilities:- Deliver Personalized Support: Provide personal care assistance and light domestic duties to clients in their homes.
- Foster Social Connections: Facilitate social support and assist clients with appointments and transport needs as required.
- Collaborative Teamwork: Work closely with the Community Nursing team to provide ongoing care and coordinate professional development opportunities.
- Assess and Respond: Assess client needs, recognize changes in condition, respond to emergencies, and coordinate with other health services as necessary.
- Travel and Flexibility: Travel to client homes; maintain a valid NSW driver's licence and be willing to travel across the community.

RequirementsTo be successful in this role, you will need:
- A current Enrolled Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience working in aged care or a related field.
- Strong communication and interpersonal skills.
- The ability to work independently and as part of a team.
- A valid NSW driver's licence.
