This is an exciting opportunity for a Registered Nurse to join our Community Nursing Service as a permanent part-time or temporary part-time staff member across multiple sites.
As a key member of our team, you will provide nursing care to people in the community setting, working closely with clients, their carers, General Practitioners and other relevant service providers to develop personalized care plans that meet individual needs.
The Community Nursing Service is an integral part of the total healthcare delivery system, providing essential support to patients in their homes and communities.
Key Responsibilities:- Provide high-quality nursing care to clients in their homes and communities.
- Develop and implement personalized care plans that meet individual needs.
- Work collaboratively with clients, carers, General Practitioners and other service providers to ensure seamless care delivery.
- Maintain accurate records and reports to support effective care planning and decision-making.
To be successful in this role, you will require:
- A current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Relevant qualifications and experience in community nursing.
- Excellent communication and interpersonal skills.
- A strong commitment to providing high-quality patient care.
