Job Opportunity: Disability Support Specialist

NSW Health is seeking a skilled and compassionate Disability Support Specialist to join our team at NSW Health in Broken Hill.

As a key member of our multidisciplinary team, the successful candidate will be responsible for providing care coordination, case management, and navigation for people with disabilities in hospital to ensure they have appropriate supports for discharge.

  • Support NDIS eligible individuals who are in hospital, at risk of hospitalisation, or where support will expedite access to NDIS services to relieve pressure on FWLHD services.
Key Responsibilities:

Requirements:
  • Tertiary qualification and 3 years clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
  • Demonstrated advanced clinical reasoning, experience, and practice skills as relevant to assessment and treatment of people with disability.
