Clinical Care Coordinator

About The Role

We are seeking an experienced and compassionate Clinical Care Coordinator to join our team. This is a part-time role that requires a Registered Nurse with a passion for delivering high-quality clinical care to elderly residents in a multidisciplinary setting.

The successful candidate will be responsible for providing holistic assessments, developing and implementing care plans, and evaluating resident outcomes. They will work closely with the Care Manager to ensure the delivery of excellent patient-centered care while maintaining compliance with Aged Care standards and regulations.

Responsibilities
  • Conduct thorough assessments of residents' physical, emotional, and social needs
  • Develop and implement individualized care plans that cater to each resident's unique requirements
  • Monitor and evaluate resident outcomes, making adjustments to care plans as needed
  • Collaborate with the Care Manager and other healthcare professionals to ensure seamless delivery of care
  • Document resident information accurately and in accordance with Aged Care documentation guidelines
  • Ensure compliance with WHS legislation and policies, as well as relevant state and federal regulations
Your Qualifications
  • Current Registered Nurse (Division 1) registration with AHPRA
  • Minimum 12 months experience as a Registered Nurse in a similar role
  • Excellent communication and interpersonal skills
  • A strong passion for working with older adults and a commitment to their health and well-being
  • Ability to maintain confidentiality and handle sensitive information with discretion
