As a vital member of our healthcare team, you will play a key role in ensuring effective discharge planning for patients. This position offers the opportunity to work in a dynamic environment, focusing on delivering exceptional patient care.
Key Responsibilities:
- Oversee and implement comprehensive discharge plans that align with the hospital's operational goals and budget.
- Facilitate smooth patient transitions by coordinating with nursing staff, allied health professionals, and medical practitioners.
- Engage with external service providers to address ongoing care needs for patients post-discharge.
- Provide essential information and support to patients and their families regarding health and nursing care options.
- Conduct early assessments of inpatients with complex care requirements to ensure timely discharge.
- Maintain accurate documentation of discharge plans accessible to all clinical staff.
Selection Criteria:
- A minimum of 2 to 4 years of experience as a Registered Nurse in an acute care setting.
- Strong interpersonal and time management skills, with the ability to thrive in a multidisciplinary team.
- Proficient in generating referrals to appropriate organizations.
- Capability to collaborate effectively with nursing unit managers and various departments.
- Problem-solving skills in a fast-paced healthcare environment.
