Senior Health Service Coordinator

This is a challenging opportunity to work as a Senior Health Service Coordinator in a clinical setting.

The successful candidate will be responsible for coordinating health services to support clients in their own homes, ensuring efficient service delivery and providing the best possible outcomes.

Key Responsibilities:

  • Coordinate daily care for new and existing clients of the SA Health Community Care Program.
  • Accept referrals from the Metropolitan Referral Unit (MRU) and ensure client information is available for timely service delivery.
  • Identify eligible clients for transfer of care to other Government funding streams, liaising with referrers, MRU or healthcare professionals.

About You

  • Hold a Bachelor of Nursing degree.
  • Have experience in acute nursing.
  • Possess community nursing skills.
  • Hold a full driver's license.
  • Have previous experience managing service contracts.
  • Have leadership experience.
  • Demonstrate strong problem-solving and time management skills.
  • Excel in effective communication.
  • Be proficient in computer skills.
  • Be able to work over a 7-day roster.
