**Job Title:** Home Care Specialist

As a Home Care Specialist, you will be responsible for assessing patients' physical abilities, medical history, and limitations, developing personalized treatment plans, and implementing therapeutic exercises and manual therapy.

You will also need to be able to collaborate with healthcare professionals, advise on mobility aids and assistive devices, and promote health, wellness, and mobility through education and support.

**Key Responsibilities:**

  • Assess patients' physical abilities, medical history, and limitations
  • Develop and implement personalized treatment plans
  • Administer therapeutic exercises and manual therapy
  • Monitor and document progress, adjusting plans as needed
  • Collaborate with healthcare professionals for coordinated care
  • Advise on mobility aids and assistive devices
  • Promote health, wellness, and mobility through education and support

**Qualifications:**

  • Qualified Occupational Therapist (OT)
  • Current AHPRA registration
  • First Aid & CPR certification
  • Australian National Police check
  • Professional Indemnity Insurance
  • Experience in community care assessment and planning
  • Valid Driver's License and reliable vehicle
  • Smart mobile phone (iPhone/Android)

**Preferred Skills:**

  • Possession of Cert IV in Training and Assessment
  • Excellent communication skills
  • Ability to work independently and in teams
  • Strong time management and organization skills
  • Commitment to ongoing professional development

Note: Residing in Western Australia is required for this position.
