Acute Patient Care Specialist – Home-Based Services

Deliver exceptional care to patients in the comfort of their own homes. As an Acute Patient Care Specialist, you will visit patients to administer antineoplastic drugs, working independently with a defined list of treatments. Prior to leaving hospital and at the point of care, medications will be checked with another qualified nurse. A strong foundation in antineoplastic drug handling and administration is essential.

This role offers permanent part-time opportunities for skilled nurses who enjoy working with acute patients in various settings. The successful candidate will work closely with medical teams, collaborating with ward staff to ensure smooth care transfer. This position requires a strong commitment to ongoing education and continuous professional development.

The key responsibilities of this role include:

  • Administering antineoplastic drugs to patients in their homes
  • Working independently with a defined list of treatments
  • Collaborating with medical teams to ensure smooth care transfer
  • Providing high-level care under the direction of the Nurse Unit Manager and medical team

Required Skills and Qualifications

To be successful in this role, you will have:

  • Current nursing registration with a relevant regulatory body
  • Valid full driving license
  • Understanding of community-based services
  • Demonstrated autonomous practice
  • Strong prioritization, time management, and organizational skills
  • Excellent communication and interpersonal skills
  • Advanced clinical assessment and problem-solving skills
  • Ability to escalate clinical deterioration
  • Commitment to ongoing education

Essential Skills

The following skills are essential for this role:

  • Minimum 3 years' experience in acute patient care or related fields
  • Knowledge of PICC and CVAD management
  • Complex wound care expertise
  • Current IV cannulation certification and venepuncture experience
  • Management of IDC and SPC, drain tubes

Desirable Skills

The following skills are desirable but not essential:

  • Chemotherapy administration
  • CVAD insertion
  • Negative Pressure Therapy
  • PleurX Drain management
  • Nutrition management, stoma care, PORT insertion
