Compassionate Care Professional

We are seeking an experienced professional to join our team as a Care Coordinator. This role involves conducting client care assessments, overseeing ongoing care planning, and coordinating personalized care aligned with client needs.
Key Responsibilities:
- Conduct thorough client assessments to determine care requirements
- Maintain accurate records of client care plans and compliance
- Coordinate and deliver high-quality care services to meet client needs
- Perform administrative tasks related to the role
Requirements:
- Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
- Experience in aged care, NDIS, or mental health case management (preferred)
- Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
- Proven experience in assessing and planning community-based care
- Excellent verbal and written communication skills with strong customer service abilities
- Ability to handle incoming calls professionally and courteously
