Case Manager - Continuing Care Team

You will provide a high standard of clinical assessment, treatment and support to clients of the integrated adult MHS and their carers, demonstrating excellence in discipline specific and recovery-focused case management skills.

This role provides ongoing specialist treatment and support for clients with serious mental illness and high disability levels, using a recovery-focused Strengths Model of Practice.

Key Responsibilities:
  • Provide clinical assessment, treatment and support to clients of the integrated adult MHS and their carers.
  • Work within the Strengths Model of recovery orientated practice.
  • Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care.
  • Undertake the assessment, provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers in line with the Mental Health & Wellbeing Act 2022 and SVMH policies and strategic direction.
Requirements:
  • Bachelor Degree in Occupational Therapy or Social Work.
  • Clinical Masters in Psychology and current Registration as a Psychologist under the Health Professions Registration Act 2005 (Vic).
  • A minimum of at least 2 years post qualification within your profession, preferably within the mental health field.
  • Experience in the provision of assessment, discipline specific intervention and case management services to consumers with a serious mental illness and complex needs and their carers within a recovery framework.
  • Highly developed interpersonal and communication skills.
  • Post-graduate qualifications in relevant area of specialty or in the process of achieving the same are desirable.
  • Current National criminal history check, or willing to obtain/ Current NDIS Worker Screening Check, or willing to obtain.
