Job Title: Clinical Care Specialist">

We are seeking a Clinical Care Specialist to provide high-quality nursing care to older Australians in their own homes.

The ideal candidate will have strong assessment and care planning skills, as well as experience in delivering person-centred care.

Responsibilities include:

  • Conducting thorough assessments to identify individual client needs
  • Developing and implementing comprehensive care plans
  • Providing wound care, catheter management, stoma care, diabetes management, and chronic disease management
Requirements
  • Current registration as a Registered Nurse with the relevant state or territory health department
  • Police check and Working with Children check
  • Valid driver's licence and access to a reliable, insured vehicle
  • Excellent communication and time management skills
