Key Responsibilities
  • Conduct in-home client care assessments and develop personalized care plans to meet their unique needs
  • Meet with clients and their families to build strong relationships and ensure their care needs are met
  • Provide some nursing care and in-home care services as needed, working collaboratively with our care team
  • Handle information requests, feedback, and general enquiries in a professional and timely manner
  • Schedule services for our clients by rostering care workers based on their qualifications and availability
  • Ensure all documentation is accurately and comprehensively completed in our databases and software programs

Requirements
  • A current AHPRA Registration
  • Qualifications in Nursing
