
Care Team Leader

We invite you to explore a dynamic role that offers the perfect blend of challenge and reward. As a Care Coordinator, you will play a pivotal part in ensuring the seamless delivery of our support services.
Your key responsibilities will include:
- Liaising with clients to understand their needs and goals, and developing tailored care plans to meet these objectives;
- Collaborating with our nursing team to implement care plans and provide exceptional customer service;
- Providing guidance and mentorship to Support Workers, and allocating resources to meet client needs;
- Processing onboarding and offboarding procedures for staff and participants, ensuring a smooth transition;
- Maintaining effective relationships with clients and Support Workers, and addressing any feedback or complaints in a timely manner;
- Contributing to our recruitment strategy, and participating in ongoing recruitment efforts to attract top talent;
- Promoting a safe work environment, adhering to our Work Health & Safety policy.
To excel in this role, you will require:
- Strong interpersonal skills, with the ability to build rapport with clients and colleagues;
- Effective time management, with a keen eye for detail and a strong work ethic;
- A Certificate IV in Community Services, Ageing Support, or equivalent qualification, and/or relevant experience in community care or similar settings;
- A sound understanding of My Aged Care, Aged Care Reforms, and Consumer Directed Care principles;
- Significant experience in the Australian Aged Care or Disability sector, with a proven track record of empathy, passion, and a caring nature;
- Highly developed organisational, administrative, and financial management skills, with the ability to determine priorities, coordinate activities, and meet deadlines;
- Experience in coordinating and allocating support services, with a focus on delivering exceptional customer outcomes;
- Exceptional customer service skills, coupled with patience and commitment to meeting client needs;
- The ability to build and maintain rapport with stakeholders, including clients, families, and healthcare professionals;
- High computer literacy, with experience using information management systems and digital software;
- Unrestricted working rights in Australia.

- Hourly award rates between $33-$40 per hour;
- Option of profit sharing;
