Care Coordinator Role

We are seeking a dedicated and compassionate Care Coordinator to join our growing home care support team.

About the Position:

  • Match clients with caregivers based on needs and preferences
  • Schedule and coordinate in-home care services
  • Communicate with families, caregivers, and healthcare professionals
  • Monitor and update client care plans
  • Ensure compliance with NDIS standards and aged care best practices
  • Provide exceptional customer service and client support

What We're Looking For:

  • 2 years Experience in aged care/disability care and knowledge of NDIS standards and Aged Care Standards preferred
  • Strong organizational and communication skills
  • Ability to multitask and work in a fast-paced environment
  • A caring and compassionate attitude
  • Proficiency with scheduling software and basic computer skills

Certificate III/IV in Individual Support (Ageing and Disability) or equivalent is desirable. The right candidate will be passionate about making a difference in people's lives and have a talent for organization and caregiving.
