About the Role

As a care coordinator, you will be the key link between our clients, their families, and our care team. Your role will involve managing schedules, coordinating care plans, and providing compassionate support to ensure that our clients receive the highest quality home care services.

Main Responsibilities:
  • Match clients with caregivers based on needs and preferences
  • Schedule and coordinate in-home care services
  • Communicate with families, caregivers, and healthcare professionals
  • Monitor and update client care plans
  • E nsure compliance with relevant standards and best practices
  • Provide exceptional customer service and client support
Requirements:
  • Experience in aged care/disability care and knowledge of relevant standards preferred
  • Strong organizational and communication skills
  • Ability to multitask and work in a fast-paced environment
  • A caring and compassionate attitude
  • Proficiency with scheduling software and basic computer skills
Qualifications:
  • Certificate III/IV in Individual Support (Ageing and Disability) or equivalent
