The successful candidate will be responsible for:
- Functioning as a clinical partner in relation to the provision of home, centre, and community-based supports and packages
- Ensuring the highest quality of care is provided to Anglican Care participants
- Ensuring alignment of technologies, methods, and practices with Aged Care Quality Standards, Charter of Rights, program requirements, and organisational policies and procedures
- Identifying individual participant needs and implementing a clinical and/or general care plan and budget in consultation with the participant's, carer, and relevant stakeholders
- Managing workload effectively through excellent planning, organisational, and time management skills
- Achieving program performance goals by working with participants to manage their individual agreed services and budgets
To be successful in this role, you will need:
- Current registration under the Australian Health Practitioner Regulation Agency (AHPRA)
- Unrestricted drivers' licence
- NDIS Worker screening clearance
- CPR Certificate (ongoing requirement for annual certification)
- National Police Check (Newcastle Anglican will consider the relevance of any reported outcomes)
- Australian work rights
- Strong knowledge and practical experience working with people who are ageing from an enablement, restorative, and wellbeing perspective
- Confident and skilled clinician, able to engage and motivate participants, families, staff, and stakeholders
- Strong case management experience (ideally in aged care) and case coordination
- Ability to conduct comprehensive assessments and develop goal-oriented care plans
The ideal candidate will have:
- Experience working with people who have dementia and their carers
- Experience and commitment to working with quality and risk management systems
- Capacity to provide education and hands-on learning in the workplace
- Experience working as part of a multidisciplinary team
