Mental Health Care Coordinator

We are seeking a skilled Registered Nurse to join our clinical team at Croydon Health Centre.
About the RoleOur Community Mental Health Services aim to support consumers in their recovery and wellbeing. The services provide a range of interventions, including intake, assessment, consultation, treatment, and care coordination. The role will be based at Croydon Health Centre, where you will work collaboratively with the Core Team to deliver high-quality patient care.
The Core Team model of care is centered around providing comprehensive clinical services to consumers. As a key member of this team, you will contribute to the delivery of these services, which include extended care coordination, targeted packages of care, and short-term care coordination. You will work closely with consumers to understand their needs and develop tailored care plans to support their recovery.
Required Skills and Qualifications- Current nursing registration with AHPRA
- Demonstrated competencies in contemporary mental health practice
- Current NSW Driver's Licence (P1 and P2 acceptable)
