Job Opportunity: Deliver Exceptional Patient Care

 

We are seeking a skilled and dedicated Registered Nurse who will play a critical role in providing patient-centered care that promotes identified health outcomes.

The Assertive Outreach Team is a tertiary level community mental health team that provides care coordinated services to consumers diagnosed with a severe and enduring mental illness.

You will work collaboratively with individuals and the multidisciplinary healthcare team to assess, plan, implement, and evaluate nursing care.

Key responsibilities include:

  • Providing evidence-informed nursing care that promotes identified health outcomes
  • Assessing, planning, implementing, and evaluating nursing care in collaboration with individuals and the multidisciplinary healthcare team

Essential Skills and Qualifications

We are looking for a highly skilled and dedicated Registered Nurse who possesses:

  • Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA)
  • Demonstrated understanding of evidence-informed practice and critical thinking
  • Demonstrated commitment to person-centered care and NSW Health Core Values
