About the Role

This is an opportunity to make a meaningful difference in the lives of individuals experiencing chronic homelessness.

We are seeking a dedicated Case Manager to join our team and provide intensive support to clients through our Journey to Social Inclusion (J2SI) program.

  • Work collaboratively with a small caseload of individuals to develop personalized support plans
  • Deliver outreach-based case management and service coordination to empower clients to access housing, health care, employment, and social inclusion opportunities
  • Build deep, trusting relationships through assertive and persistent engagement
  • Partner with local organizations and First Nations workers to provide culturally safe support
What You Bring
  • A relevant tertiary qualification in Social Work, Psychology, Community Services, or a related field
  • Demonstrated experience working with people with complex needs, including homelessness, mental health, and substance use
  • Experience in trauma-informed care, case management, or advocacy
  • A values-driven approach to supporting people from diverse backgrounds
  • A current Employee Working with Children's Check and VIC Driver's Licence
