Key Community Mental Health Nurse Role

As a key member of our team, you will be responsible for:

  • Case Management: Conduct comprehensive assessments, develop tailored care plans, and provide ongoing support to clients on their recovery journey.
  • Community-Based Care: Deliver mental health services in home, clinical, and community settings, ensuring accessible and effective care delivery.
  • Crisis Intervention: Provide immediate support to individuals in distress, prioritizing safety and stability.
  • Collaborative Approach: Work closely with a multidisciplinary team, including psychiatrists, social workers, and primary care providers, to ensure cohesive care delivery.
  • Advocacy & Support: Educate clients and their families, connecting them with essential resources for long-term care.

We are looking for individuals who possess:

  • 5+ years of experience in mental health case management
  • Current AHPRA registration as a Registered Nurse (Clinical Nurse / Clinical Nurse Consultant)
  • Ability to work autonomously in regional & remote settings
  • Passion for delivering high-quality mental health care to diverse communities
