About the Role:

We are seeking a compassionate and experienced Support Worker to provide high-quality support to individuals with daily living skills, assist with social and recreational activities, and empower participants to achieve their goals.

Key Responsibilities Include:

  • Build rapport and collaborate effectively with staff/management, participants, their family, and friends to achieve a positive outcome and ensure a smooth service delivery flow throughout the participants' plan.
  • Assist with liaising with participants' families and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide a medium of solid communication.
  • Increase participants skills with your active support and promote independence by enhancing their skills and personal development.
  • Provide quality and professional support in line with participants plans, needs, and wants.
  • Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle.

Requirements:

  • You must have experience in providing In-Home Support and hold a current driver's license and own car to use on shifts.
  • Be fully vaccinated for Covid-19.
  • Have at least 1 year of experience in home care and complex care.
