As a Support Coordinator, you will play a vital role in helping individuals with disabilities navigate the National Disability Insurance Scheme (NDIS) and achieve their goals.
About the RoleThe NDIS offers assistance to people with a broad range of physical, intellectual, and psychosocial impairments. You will work directly with participants and their support networks to understand their NDIS plan, identify suitable services within the community, and support them to communicate their goals and aspirations to their stakeholder team and the NDIA.
- Support and communicate participants, their families, and their formal/informal support network to understand their NDIS plan;
- Identify the most appropriate services within the community to suit an individual's needs;
- Support participants to communicate their goals and aspirations to their stakeholder team and the NDIA;
- Attend home and community visits with participants;
- Undertake general case management of participant portfolios; and
- Manage and monitor individual NDIS Plan budgets.
To be successful in this role, you will need to have excellent communication skills, an eye for detail, and the ability to think outside the box. You will also need to be organised and able to work independently as well as part of a team.
