Low-Cost Accommodation Support Program Case Manager Role

This is a challenging and rewarding role for an experienced Case Manager to join our Low-Cost Accommodation Support Program.

The successful candidate will deliver on-site and outreach-based support to vulnerable adults and older people living in low-cost accommodation or public housing.

Our program focuses on addressing complex health and psychosocial needs, isolation, and housing insecurity through short-term, goal-directed case management.

Key Responsibilities:

  • Assist clients to sustain tenancies
  • Access health and social services
  • Reconnect with their community

Requirements:

  • Experience in case management or similar roles, ideally working with older people or those facing multiple barriers to service access
  • Skilled in engaging people through assertive outreach approaches
  • Knowledgeable in housing and homelessness systems, aged care, and disability services
  • Strong communicator with ability to advocate, collaborate and navigate complex service systems
  • Commitment to social justice, cultural safety, and continuous improvement
