Perinatal Mental Health Navigator

About the Role
We are seeking a part-time Care Navigator to be based at Ngala, Kensington. The successful candidate will work with a child-centred, strength-based and collaborative approach to provide mental health navigation services to new and expecting parents.
Key Responsibilities
- Identify state-wide PIMH support services and referral pathways.
- Build productive relationships and stakeholder engagement with key individuals and groups.
- Connect and care specific tasks including intake, screening, and triaging and allocation of families to local services.
- Ensure the effective co-ordination and integration of services, timely, accurate and quality record keeping, and project monitoring.
About You
You will possess a Bachelor or Diploma in Social Work, Nursing or Psychology, and current Registration with the Australian Health Practitioners Regulation Agency (AHPRA). You will also have knowledge and understanding of mental health issues in antenatal, birth and the postnatal period, and demonstrated experience of collaborative person-centred care and treatment planning and working with families.
