Lead In-Home Nursing Professional
This rewarding role offers the opportunity to make a real difference in clients' lives by providing essential in-home nursing care. As part of our Home Care Package Program and National Disability Insurance Scheme, you will be responsible for the care of clients in their own home.
- Main Responsibilities:
- Provision of in-home nursing care
- Undertaking assessments to support clients accessing services and support
- Working collaboratively to develop and implement individual care plans for complex clients
- Supporting our Clinical Governance Framework by providing expert advice to other staff regarding care and incident response for clients with complex needs
- Proactively contributing to the development and implementation of policies and procedures to enhance the services provided to all clients
- Leadership for the nursing service within the identified region/s and support and oversight of enrolled nurses providing in-home nursing care
Requirements:
- Bachelor of Nursing Degree
- Minimum 5 years of nursing experience
- Minimum 3 years community (or aged care) Nursing experience (Desirable)
- Current C class Driver's license (QLD)

- Can assess client needs and develop appropriate plans for interventions and treatment
- Can develop and provide reports regarding clients' needs and conditions
- Can professionally work with internal and external stakeholders to meet client needs
