Job Title: Clinical Nurse Consultant - Heart Failure and Transplant

This is an exciting opportunity to work collaboratively with a multidisciplinary team at The Sydney Children's Hospitals Network. As a Clinical Nurse Consultant Grade 2, you will provide safe, compassionate care to children and families with heart failure or pulmonary hypertension.

The role involves undertaking primary case management of children and families, coordinating heart transplant assessments, and participating in on-call services for donor referrals and transplant coordination.

You will collaborate with all members of the team and contribute to interdisciplinary clinical decision making, including appropriate referrals to stakeholders as required.

The CNC2 will act as a resource for nursing staff regarding post-operative care of heart transplant patients, support and lead review of nursing practices, and maintain databases.

In addition, the role involves identifying learning needs of patients and their families and educating caregivers to acquire the information needed to care for their child throughout the transplant continuum.

Key Responsibilities:
