Nurse Practitioner

We are seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team at Country Health Connect. As a key member of our healthcare team, you will provide expert clinical nursing care to patients and their families, ensuring a patient-centered approach.
Key Responsibilities:
- Deliver high-quality clinical nursing care to individual patients and groups
- Provide clinical leadership to nursing and midwifery staff, fostering a collaborative and supportive environment
- Coordinate and lead projects and programs aimed at improving service outcomes
- Accept accountability for nursing practices and outcomes, ensuring adherence to policies and best practices
- Assist the Regional Manager in service management duties, including performance management, staffing, and financial planning
Requirements:
- Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia, holding a current practising certificate
- Minimum of 3 years post-registration experience as a Registered Nurse/Midwife
- Advanced nursing practice experience in managing specific patient/client groups
- Background in developing clinical policies, procedures, and models of care
- Experience supervising nursing students and less experienced staff
- Proficient in Microsoft Office Suite and other relevant software
